We founded Living Provenance to give every family what ours needed most: one place that holds the whole story, and shows where each part came from.
For years, we carried our son's story from place to place: hospitals, specialists, Therapeutic providers and Educational teams. Every new door meant telling it again, and hoping we remembered the details that mattered.
What we didn't have was one place that held all of it, and could show us where each fact came from.
So we built one.

Sam was our dog. She was always by our son's side, and in her own way, she always spoke up for him. When we built something to hold his whole story and help us speak up for him, it could only have one name: SAM, Someone Always with Me.
Now we're building it for every family.
It's our name, and SAM's promise. SAM is built so every answer can be traced to the exact Medical Record it came from.
